New Patient

Please bring the following with you to your appointment:

• Completed new patient paperwork packet.
• Government issued photo ID.
• Insurance card(s).
• Any imaging studies you have had (MRIs, X-rays, CT scans). We ask that you bring both the films and the report. 
• A list of your current medications and their doses.
• If you have had spine surgery elsewhere, please bring a copy of the operative report (if available).
