Items to Bring

Please bring the following with you to your appointment:

Completed new patient paperwork packet.
Government issued photo ID.
Insurance card(s).
Any imaging studies you have had (MRIs, X-rays, CT scans). We ask that you bring both the films and the report.
A list of your current medications and their doses.
If you have had spine surgery elsewhere, please bring a copy of the operative report (if available).
